血必淨對大鼠急性胰腺炎保護作用機制探討

【摘要】  目的 探討血必淨對大鼠急性胰腺炎的保護作用及機制。方法 SD大鼠48只, 隨機分為假手術組、胰腺炎組和治療組,每組再分為4 h、8 h亞組。假手術組僅行開關腹手術,胰腺炎組以牛磺膽酸鈉造模,治療組以血必淨干預。光鏡觀察胰腺組織病理改變,免疫組化法測胰腺組織腫瘤壞死因子(TNF)-α、干擾素(IL)-1β。結果 治療組胰腺組織損傷較胰腺炎組明顯減輕,治療組胰腺組織中TNF-α、IL-1β含量顯著低於胰腺炎組 (P<0.01)。結論 血必淨可能通過抑制TNF-α、IL-1β的產生減輕胰腺炎症。

Abstract: Objective To investigate the protective effect of Xuebijing (XBJ) on acute pancreatitis in rats and its possible mechanism. Methods 48 SD rats were randomized into 3 groups: sham group, pancreatitis group and XBJ group, each group sub-divided into 4h and 8h subgroups. In the sham groups, the rats only underwent opening and closure of the abdomen; the pancreatitis groups received taurocholate sodium administration to establish animal model and the treatment groups were intervened by Xuebijing. The pathologic changes of pancreatic tissues were observed by optic microscopy. The expressions of tumor necrosis factor (TNF)-α and interleukin (IL)-1β in pancreatic tissues were determined by immunohistochemical methods. Results The histomorphological changes of the pancreatic tissues in XBJ group were much more relieved than those in the pancreatitis group. The expressions of TNF-α and IL-1β were significantly lower in XBJ group than those in the pancreatitis group (P<0.01). Conclusion XBJ could relieve acute pancreatitis in rats by inhibiting the production of TNF-α and IL-1β.

腫瘤壞死因子α(TNF-α)、干擾素1β(IL-1β)是引起組織細胞損傷的重要細胞因子,兩者均參與急性胰腺炎的病理過程。近年來已有關於血必淨對血液中細胞因子影響的報道[1-2],但血必淨對急性胰腺炎時胰腺組織TNF-α、IL-1β的表達有無影響尚未見報道。本實驗旨在通過觀察血必淨注射液對大鼠胰腺組織TNF-α、IL-1β表達的影響,探討血必淨對大鼠急性胰腺炎的保護作用及可能機制。

1 材料和方法

1.1 動物模型與分組 將SD大鼠48只(體重200~250 g, 由徐州醫學院實驗動物中心提供)隨機分為假手術組、胰腺炎組和治療組(n=16),假手術組僅行開關腹手術,胰腺炎組經胰膽管逆行注射5%牛磺膽酸鈉(2 ml/kg,美國Sigma公司)製作胰腺炎模型,治療組在胰腺炎模型基礎上予血必淨干預。術前3天,治療組經尾靜脈注射血必淨注射液4 ml·kg-1·d-1(天津紅日藥業股份有限公司生產);假手術組、胰腺炎組同時間點給予等量生理鹽水。3組分別於制模術後4h、8 h時點處死大鼠,取胰腺標本進行檢驗。

1.2 組織病理學檢查 切取胰腺標本製成石蠟切片後行蘇木精-伊紅染色,光鏡下觀察胰腺組織病理學變化。

1.3 TNF-α、IL-1β表達檢測 免疫組化染色法(試劑盒均購自武漢博士德生物工程有限公司)測胰腺組織TNF-α、IL-1β表達,結果應用計算機進行影象灰度分析,以積分光密度值(D)作計量分析,每張片選4個視野。

1.4 統計學處理 使用SPSS 11.0統計軟體進行統計,資料以±s表示。計量資料採用t檢驗,計數資料採用χ2檢驗,P<0.05為差異有統計學意義。

2 結 果

2.1 組織病理學結果 假手術組胰腺組織肉眼及鏡下結構無明顯異常。胰腺炎組肉眼觀胰腺明顯腫脹,伴有出血壞死;鏡下見間質血管擴張、淤血,灶狀胰腺腺泡與脂肪壞死,大量炎性細胞浸潤,紅血球滲出,胰腺細胞結構消失。治療組肉眼病變及鏡下觀察組織結構破壞明顯減輕。

2.2 免疫組化測定 治療組TNF-α、IL-1β含量顯著低於胰腺炎組(P<0.01 ),見表1、2。表1 各組TNF-α的.D值表2 各組IL-1β的D值(n=8,±s)與假手術組比較:*P<0.01;與胰腺炎組比較:△P<0.01
